Clear Path Rehabilitation, LLC is a Massachusetts based clinical telehealth provider for Commonwealth residents (21 and older) with a substance use disorder. We use technology to deliver non-emergent specialized healthcare services remotely and are available as you are to ensure service delivery is fast, secure, and efficient.
Our mission:
Our mission is to support Commonwealth residents (21 and older) and their families living with substance use disorder and improve their level of function to live successfully in the community. We incorporate assessment and care planning processes and collaborate with families and providers to improve both outcome and individual health.

The goal of telehealth is to improve access to and delivery of therapeutic services ensuring all receive the best possible care regardless of location or transportation. This process allows people to connect with providers most conveniently and eliminates wasted time in travel.

Scheduled Appointments:
The core service at Clear Path Rehabilitation, LLC; scheduled non-emergent one-to-one clinical telehealth to assist individuals in attaining or supporting their recovery in the community. Our experienced team will walk you through the initial processes which result in the development of an individualized plan to meet our shared goals. We pride ourselves on our exceptional service delivery.

Consultation:
Our clinicians are available to consult regarding individual referrals, availability, and appropriateness of Clear Path Rehabilitation, LLC's clinical telehealth services. With this service, we ensure all processes are simple, seamless and handled in a timely manner.
Peer Recovery Coaching:
Individuals may be connected with a Recovery Coach, who is a person that has lived experience with recovery and has been trained to help others to gain hope, explore recovery, and achieve life goals. Their role is to support the voices and choices of those they are connected with. An individualized continued care plan is developed for each individual and includes continued weekly contact with the recovery coach, community acclamation and socialization, and introduction to recovery support groups.
